- The annual registration health form must be completed every school year. This form is now online and can be accessed through your Aspen account. This form must be completed for your child to receive any over the counter medication at school such as tylenol, ibuprofen, benedryl, etc.

- An updated immunization record and history and physical (within the past year) are required upon admission to HMHS.
