If your student has life threatening allergies, asthma, diabetes, or seizure disorder, please have your child's health care provider fill out the applicable forms below, or similar standard forms used by the health care providers' office. A new form is required by law each school year. Please be sure that the submitted forms have the signature of both the physician and the parent/guardian.
